Irreparable tears of the subscapularis (SS) tendon are difficult to manage and represent a challenge for surgeon, especially in young active patients. They associated with horizontal imbalance shoulder, causing pain limitation internal rotation. Historically, alternative these patients has been transfer pectoralis major, all its variations, total or partial, up under conjoint tendon. However, this mechanical disadvantages, related vector traction, because it originates anterior region chest. In 2013, Elhassan colleagues demonstrated cadavers technical feasibility neurological safety performing transfers latissimus dorsi (LD) lesser tuberosity reconstruct irreparable lesions subscapularis. This option, compared alternatives, superior biomechanical advantages such as similar originating from lower posterior thorax, addition involving synergistic muscle action. early 2016, Kany first published study 5 undergoing arthroscopic assisted LD SS transfer, promising results.
